Recommended Schedule for Routine Immunizations:
  • 2 Months          Pediarix*, Pedvax, PCV13, RV
  • 4 Months          Pediarix*, Pedvax, PCV13, RV
  • 6 Months          Pediarix*, PCV13, RV, MMR
  • 12 Months        Varicella, Pedvax, PCV13, MMR, Hep A
  • 15 Months        DTaP, MMR
  • 18 Months        Hep A
  • 4 Years             Varicella, Kinrix**, MMR
  • 11-12 Years      DTaP, MCV4, HPV
  • 15-18 Years      MCV4, HPV

* Pediarix = combination vaccine of DTAP/IPV/Hep B
** Kinrix = combination vaccine of DTAP/IPV

Influenza (Flu) Vaccine Update

Flu season is upon us. We are concerned about your health and encourage everyone to get vaccinated annually against influenza. It is critical that everyone who is able receives a flu vaccine. It’s essential that Active Duty service members, family members and other beneficiaries are well-protected from influenza. Please see the guidance below for getting the flu vaccine at USNH Naples:  

Make your appointment for the flu vaccine by clicking HERE
  • Select the green "Next" button at the bottom of the page
  • Select the "Annual Flu Vaccine" option 
  • Click the date(s) highlighted in blue to select a time for your flu shot. 
All individuals receiving the flu vaccine from USNH Naples must complete the "Defense Health Agency (DHA) Form 116 Pediatric and Adult Screening and Immunization Form."

The form can be downloaded by clicking HERE - a fillable PDF file will begin downloading. Complete sections 1-6, print the form and bring it to your flu vaccine appointment.  

The seasonal influenza vaccination is required for all DoD uniformed personnel who are not medically or administratively exempt. Click HERE to read the 2021-2022 NAVY INFLUENZA VACCINATION AND REPORTNIG POLICY (NAVADMIN 235/21).  

NOTE; If you receive a flu vaccine from a non-DoD provider, refer to your service policy guidance for recording the vaccine in your shot record. You'll need the following information from your vaccine provider: 
  • Location and date the vaccine was given
  • The vaccine name or code
  • Manufacturer name
  • Lot number
